Course Title: Capturing Critical Information in the Incident Investigation Process

Course Type: Webinars

Objective: This webinar focuses on the importance of gathering critical information to conduct thorough incident investigations. It discusses techniques to collect proper data and the need to determine contributing factors and root causes. In addition, the webinar provides information on the items to document and what to do once the incident investigation is complete.

Pre-Requisites: This introductory-level presentation is beneficial to personnel wanting to learn more about the information to capture in the incident investigation process. Personnel gaining the most benefit from this presentation may include: safety professionals, directors and upper management, managers and supervisors, front line employees, and others involved in any aspects of the incident investigation process (Collateral Duty Safety Officer [CDSO], Additional Duty Safety Officer [ADSO], Unit Safety Officer [USO], Unit Safety Representative [USR]).

Maxwell Bassett, DoD SMCX Senior SOH Professional

Mr. Bassett has been with the DoD SMCX for 3 years. A former Deputy Safety Program Director for Navy Region Southeast and Naval Flight Officer, he spent the 12 prior years mentoring DoD STAR sites, serving as a Special Government Employee (SGE), and overseeing the safety programs for 1/3 of all stateside Navy Shore Installations.

Ben Lacy, DoD SMCX ES&H Manager

Mr. Lacy has been supporting the DoD SMCX for nearly five years. After retirement from the U.S. Army as a Colonel and Master Army Aviator, he has focused on assisting organizations with SMS implementation. Mr. Lacy is a graduate of both FAA and NTSB accident investigation courses, with extensive accident investigation experience. He currently serves as a committee member on the National Academy of Science’s (NAS) Transportation Research Board (TRB) Aviation Safety and Security (AV090) Committee
